By booking this consult, you are agreeing to the following Terms and Conditions. Please see full T&C’s below.
Once you have read the Terms & Conditions below, you may book your ‘In Person’ or Via Zoom’ appointment by clicking the appropriate Book Consult button below. Upon successful calendar booking, you will receive email confirmation.
This agreement is made:
BETWEEN: Jacinta O’Connor ABN: 19 782 452 405 ("Practitioner")
AND: The booking name upon payment of consult (“Client”)
SCHEDULING & CANCELATIONS
If the Client needs to cancel or reschedule the appointment, the Client must do so 48 hours in advance; otherwise, the Client will forfeit that appointment and will not have an opportunity to reschedule.
PAYMENTS & REFUNDS
The Client understands that the cost of the consult is $140AUD for 60 mins (one family member) or $80AUD for 30 mins for (one family member).
Payment is due at the conclusion of the consult. Due to the nature of scheduling and preparation for consultations, the Practitioner does not offer refunds.
PRACTITIONER INFORMED CONSENT & DISCLAIMER
The services of the Practitioner, as a Naturopath and Nutritionist, are not intended to be a replacement for or a substitute for those of a licensed physician.
The practitioner does not practice medicine nor diagnose or treat diseases or medical conditions. No comments or recommendations should be construed as a medical diagnosis.
The Practitioner will advise you, if needed, to be under the care of a licensed physician and encourage open communication between her and your current medical practitioner. It is important to inform your healthcare provider of changes in your physical condition.
Changes in nutrition and lifestyle may alter your need for medication. It is important that the Practitioner is informed of your use of medications, prescription and non-prescription supplements.
Certain foods and supplements can interfere with prescribed medications. You always have the right to refuse any proposed recommendations. You, the client, must make all the decisions relative to your own health and wellbeing.
By booking this consult with the practitioner, you acknowledge and understand nutrition is considered an inexact science and results obtained are not always constant or predictable.
Additionally, you acknowledge that you understand this document and its contents.
TESTING & SUPPLEMENTS
The Client is welcome to bring supplements and test results to their appointment. Subsequent emailed copies of test results and supplements will be addressed at the next consultation.
EMAIL & TEXT REQUESTS FOR ADVICE
Included in the Consultation Fee is answering questions the Client may have in relation to the Practitioner recommendations, for seven (7) days after the Client receives recommendations.
Emails or texts requesting advice outside the seven (7) day period, will not be answered. A 15 or 30 minute consultation can be booked for this purpose.
CONFIDENTIALITY
The Practitioner will keep the Client’s information private, and will not share the Client’s information to any third party unless compelled to by law.
